Application form

This is an application form and must be completed together with a parent/legal guardian. Only fully completed application forms will be evaluated.

For players that have not yet been participating with NF Academy, please complete the form “First time with NF Academy”.

If you are a player that has already participated in a program with NF Academy please complete the form “Former NF Academy player”.

FIRST TIME WITH NF ACADEMY


Video requirements

- Go to Wetransfer, upload the video (and if you have, your football CV) and get the link for the files. Paste the link in field below.
- Must be from a game situation, between 3-6 minutes where we clearly see the player in question.
- Video can be filmed using a phone camera, but cannot be filmed too far away.
- Describe who we should evaluate in the video (ex: colour of clothes, jersey number, hair colour, shoe colour)
- Remember to write the name of the player in the Wetransfer message field.
